Sport Imagery Questionnaire
Situation 1: Practicing Alone
In selecting a specific skill in a sport, I will choose running a play action pass play as a quarterback for my football team. I am imagining myself performing this skill at the play where I normally practice, which is on the field at the park without anyone else present. It is very quiet and all I can hear are the planes overhead, barking dogs, and the rustling trees. I am running the play as if my teammates were there and going through every foot movement. I feel as if everything I am doing is right.

Situation 2: Practicing With Others
For the second situation, I am still performing the same skill, running a play action pass play, but now I am doing it with my coach and teammates present. This time as I am running the play, I open up with the wrong foot and run the play all backwards. I am embarrassed for making a mistake since I am supposed to be a leader and be able to guide everyone through the plays that I should know very well.

Situation 3: Watching a Teammate
For the third situation, the pressure is taken off of me and now I am watching a teammate perform a specific skill. This specific skill involves my teammate who is the right tackle on the offensive line and her skill is the block a linebacker from getting into the backfield. This time, my teammate performs this skill unsuccessfully in an actual game and causes our team to lose yards. I am well aware of the mistake and am upset, but there is so little time to dwell over it so I let her know to hold her block so we can run the same play again to gain yards.

Situation 4: Playing in a Contest
For the fourth and final situation, I am imagining myself performing in a competition or game. This time I am performing my skill at my best and the fans and my team are cheering me on. I feel great and very energized and continue to successfully run my play action pass plays. I can hear everyone's voices and my name being tossed around, but I am still focused on my team and winning the game.

Scoring
A. Visual: 3 + 4 + 5 + 5 = 17
B. Auditory: 2 +3 + 3 +4 = 12
C. Kinesthetic: 4 + 4 + 3 + 4 = 15
D. Mood: 3 + 4 + 5 + 5 = 17
As far as scoring goes, the higher the score (20 being the highest) the more skilled you are in the certain area, and the lower the score (4 being the lowest) the less skilled you are in the certain area which means you need to work on those aspects more in your imagery. In evaluating my own scores, it looks like I am pretty skilled in the visual and mood aspects, fairly skilled in the kinesthetic aspect, and not as skill in the auditory aspect.
